Information for New Clients

On your first visit, we will need to complete some forms.  You can print them now and review them prior to your appointment. 

The files you will be printing are:

· An authorization to release information to doctors, therapists and/or family members

· A client registration form to give me background information about you

· A list of my fees

The documents are saved in pdf format.  Simply click on the link below.
