Insurance
Reimbursement
The client is asked to file for insurance reimbursement. Some insurance companies require prior approval and a physician’s referral in order to obtain reimbursement. Please contact your insurance company prior to your first appointment to obtain reimbursement details. The appropriate paperwork for insurance reimbursement will be provided at each session, upon payment.
Phone
Sessions
Phone sessions are available for clients who are out of town, have a crisis between regularly scheduled appointments, or during a natural disaster. For long standing clients who have a last minute emergency and cannot attend a scheduled session, a phone session may be requested at the scheduled time. For therapeutic reasons, Ellen may decline a last minute request to change from an office visit to a phone session. It is the responsibility of the client to initiate the call for a phone session.
24-hour cancellation notice is respectfully required. You will be charged for the missed appointment unless notice of cancellation is received 24 hours in advance of the scheduled time. This fee is waived in the case of emergency hospitalization or natural disasters. The fee for missed appointments is due within 7 days or at your next appointment, which ever comes first.
If you repeatedly do not show for appointments or have repeated late cancellations, this may result in termination of Ellen’s services.
